U kunt via deze weg alleen herhaalmedicatie aanvragen die al eerder door ons is voorgeschreven.
Lees hieronder de informatie over herhaalrecepten. Herhaalrecepten van apotheken Benu Crooswijk, Ramleh, Exler Noordplein, Rozenburg en Spannhof mogen direct bij de apotheek worden aangevraagd. Voor andere apotheken, of als het gaat om bijzondere medicijnen kunt u contact opnemen met onze praktijk, of bestellen via onderstaand formulier.

LET OP: herhaalrecepten zijn alleen mogelijk voor medicatie die wij u hebben voorgeschreven, en die u moet blijven gebruiken. Medicatie die u van de specialist heeft gekregen zal de specialist moeten blijven voorschrijven totdat de behandeling is overgedragen aan de huisarts.
Aangevraagde recepten worden op werkdagen om 12:00u door onze praktijk verwerkt, en naar de apotheek gestuurd.

PLEASE NOTE: Repeat prescriptions are only available for medication that we have prescribed to you, and that you need to continue using. Medication prescribed by a specialist should continue to be prescribed by the specialist until the treatment has been transferred to us.
All prescriptions we have received before 12:00 will be sent to the pharmacy the same day.
